Community Care Services

HSH is a primary care provider. We provide services very similar to those that you would expect to receive from your community GP practice, and we refer to these as ‘Community Care Services’. These include:

  • Routine Check-ups
  • Phlebotomy
  • Vaccinations and Immunisations
  • NHS Health Checks
  • Management of Long-Term Conditions
  • Management of Substance Misuse
  • Blood Borne Virus Screening and Treatment
  • Sexual Health Screening, Treatment and Advice
  • Contraception Services
  • Cervical Cancer Screening
  • Health Promotion
  • Referral to Secondary Care

Advanced Clinical Care

Due to the restrictions that our patients have on accessing additional or advanced healthcare services, we try to deliver as much of this in-house as we are able to. This includes:

  • Diabetic Education
  • Ear Care
  • Pulmonary Rehabilitation
  • Spirometry
  • Minor Operations
  • Joint Injections
  • Long-acting Reversible Contraception (LARC) Fitting

Emergency / Acute Care

We care for individuals who are often very unwell – they may have chronic, poorly managed health conditions, complex health needs, and often display behaviours that are harmful to their health. To manage this and to keep our patients safe and well, we also provide:

  • Basic Life Support
  • See and Treat
  • Wound Care and Management
  • Substance Misuse Detox Monitoring
  • Admission to Accident & Emergency

Support Services

All of this clinical care cannot be provided with the essential work that our support team do. This includes:

  • Medical summarising and clinical coding
  • General administration
  • Digital systems and IT support
  • Information governance
  • Finance and payroll
  • Recruitment and on-boarding
  • Human resources
  • Employee relations
  • Risk management
  • Patient safety
  • Health and safety
  • Training, learning and development
